I understand that completing an application does not mean that I will be or not be able to adopt a pet from Firehouse Chihuahuas.
I understand that a chihuahua is not a fashion accessory or a status symbol. I will support Firehouse Chihuahua's efforts to insure that the new home is a permanent one

2) Discuss my current and previous pets with my veterinarian.
(3) Make a scheduled phone consultation to have a verbal update from the new owners to check the status of the puppy.
I understand that Firehouse Chihuahuas is using this information to determine the best home for their puppies. By in NO way is this form bias against any race, creed, nationality, handicap or income based discriminatory in nature.
I understand that all pets are to be spayed or neutered as agreed upon by contract or registered by CPD as such. Reselling is in violation of the contract, all those that are unable to "keep" their puppy must return them to Firehouse Chihuahuas.
I am able to afford annual veterinary care including vaccinations and prescribed medications for a pet, as well as emergency care, if needed.
